Our care coordinator team serves as your personal advocate and is responsible for the oversight and development of your personal care needs. Upon initial contact an experienced care coordinator will conduct an in-depth screening based on the living situation and care requirements for yourself or loved one. A thorough in-home assessment then takes place which focuses on the personal needs of the individual and with consideration given to the family’s wishes and concerns. After which a customized care plan is created so the cares performed are consistent and complete.

During your initial assessment, our attentive staff will inquire about the various care options your loved one may or may not require, including:

Certified Care Personnel

  • Activities of Daily Living (ADL)
  • Vital signs (temperature, pulse, respiration and blood pressure)
  • Universal precautions procedures

Non-Medical Care/Activities of Daily Living (ADL)

  • Ambulation assistance
  • Grooming/bathing/dressing
  • Light housework/meal preparation
  • Shopping/errands
  • Companionship

For your convenience, we are licensed for Medicaid reimbursements; however, private pay situations are welcome and many long term care insurance providers are accepted. Often, we advise our elderly care client families to consider an hourly package, which is favored for its long-term value.
