Who Does a PCW Work With?

Typically a caregiver works with the Care Team or a RN Supervisor to fulfill the tasks on a careplan specifically designed to meet the clients needs.

What does a PCW do?

An easy to read and understandable careplan specifically drafted for each client guides the PCW on how to perform cares.

The PCW performs client/patient care activities and related supportive services in caring for the personal needs, safety, and comfort of client/patient as assigned. They also may assist in providing a physical, social, and psychological environment which will allow the client/patient to achieve the highest level of functioning in their home.

Qualifications:

  • Be at least 18 years old
  • Completed a training program that provides the individual with the skills required to perform personal care services
  • Ability to read, write, speak, and understand English and the ability to follow orders.
  • Ability to perform essential functions of the job with or without reasonable accommodations
  • Ability to provides covered PCW services according to the client/patient’s care plan
  •  

  • Responds appropriately to client’s needs
  • Reports changes in the client’s condition to the supervising RN
  • Is not a consumer of Personal Care
  • Meet all health requirements and be cleared of criminal background check imposed by law
  • Must be U.S. citizen or have valid Alien work permit from immigration office
  • Preferred some experience with home healthcare and Long-term care
  • Ability to comprehend applicable federal and state regulations

Duties and Responsibilities:

Perform the duties required of you and assigned to you by the RN Supervisor, including but not limited to:

  • Bathing in, or out of bed- full or partial;
  • Dressing and undressing;
  • Oral hygiene;
  • Skin care;
  • Hair care;
  • Shaving;
  • Assistance with food, nutrition and diet activities;
  • Ambulating/transferring/muscle strengthening exercise to maintain client’s optimal level of functioning;
  • Positioning and turning;
  • Apply hearing aids, glasses, dentures, and prosthetic devices as instructed;
  • Monitoring and reporting changes; Assure a safe, clean, and comfortable environment; and Tidying room and bed making.
  • Accompany client/patient grocery shopping and to medical appointments
  • Provide care to client/patient while constantly on feet and regularly engages in lifting bending, stooping, twisting, pushing and pulling
  • Bowel and bladder care
  • Application of non prescription lotions, powders, or ointment; and treatments such as heat lamp and foot soaks
  • Possess special interest in, and a positive attitude about working with home care clients and the elderly
  • Reports suspected client/patient maltreatment in accordance with agency policy and applicable Vulnerable Adult Abuse law
  • Perform other duties as assigned and consistent with level of preparation and experience; and additional duties as deemed necessary
